Agreement (Must Complete PRIOR to Appointment)

I have read and understand the terms and conditions on the “Tips for your Checkride” page of this website.

I agree to the fee and additional fee structure.

I have accomplished the checklist in the front of the appropriate Practical Test Standards book.

I meet all eligibility requirements and aeronautical experience requirements for the certificate/rating being sought.

My instructor is aware of the date and time of my checkride appointment, and has signed all applicable endorsements.

My IACRA is complete and correct.

This form must be filled out by you and your instructor no less than 24 hours prior to your appointment time, then it will automatically forward to me. Otherwise, your appointment will be cancelled and a rescheduling fee will apply.
